There are two kinds of management plans for patients with a chronic disease:
- A GP Management Plan
- A Team Care Plan
A GP Management Plan is a plan developed by your GP for patients with long term medical conditions such as Asthma, Diabetes, Heart Disease, Arthritis, Etc who require a structured approach. This enables GP’s to plan the care of patients with complex conditions requiring ongoing care.
Together you will decide:
- What your health care problems and needs are
- What results you would like to achieve through the Plan
- What, if any, other health care and community services you need
Once the plan GP Management plan is developed you will discuss with your doctor the plan of action, and goals needed to be achieved. You should review the progress of these goals with your doctor every three months.
If you would like a carer, family member or someone else present for these appointments, please tell the nurse or doctor beforehand.
A Team Care Plan is available for patients who have a chronic or terminal condition and complex care needs requiring ongoing care from a multidisciplinary team comprising their GP and at least two other health or care providers . This can be done in addition to a GP Management Plan.
Your doctor can provide you with up to Five (5) Medicare Rebateable Referrals per calendar year for services to Allied Health Providers.
What do these plans cost?
At Goonawarra Medical Centre Medicare covers the cost of each plan and Referrals. There is no out of pocket cost to patients.
